NEIL WALKER
“Joy of the Game” Baseball Camp
Ages: 7 through 12 (“baseball age” as of the 2015 baseball season)
Date: August 6, 2015
Time: 9:00 – 12:30
Location: Fairview Park in South Fayette
Cost: $75 per camper
Each camper will receive a camp T-shirt; an autographed photo card from Neil; a photograph with Neil (no autographs on those please); and small-group instruction with an anticipated ratio of 6 campers to each counselor/instructor. Neil will be present and active throughout.
THIS CAMP WILL SELL OUT. We must limit attendance consistent with the capacity of the facilities and the instructors chosen by Neil, in order to assure a positive experience for every camper.
By registering for this Camp, I understand that in the event of injury, Fund representatives may obtain medical care from medical personnel on site at the camp or elsewhere if I am not available. I further agree to hold harmless the Fund, its directors, officers, volunteers or representatives from any liabilities resulting from my child’s participation in the Camp. And I also agree to permit the use of photographs or video taken at the camp to be used by the Fund for purposes consistent with the promotion of its missions, including without limitation its website and Facebook pages.
